What We Do


WE CAN POINT YOU IN THE RIGHT DIRECTION

The first step to access government-funded aged care services is to get assessed. You can apply for an assessment online or you can call My Aged Care on 1800 200 422.

The online application is quick and easy but if you would prefer, there are contact centre staff that will talk with you about your needs and circumstances, and they can process your application- you just need your Medicare card! If your application is successful, an assessor will contact you to arrange a face-to-face assessment.

Step One

The first step is an eligibility check. To do this, they will ask you about; your health; how you’re managing at home; and any support you’re currently receiving.

Your answers will help them understand your needs and whether you are eligible for a face-to-face assessment.

If you are eligible, they will then ask you for a few details to complete your application.

My Aged Care will keep a record of your application, so you won’t have to provide the same information again at your assessment.

Step Two

If you need support, a family member, friend, or carer can support you to apply online or over the phone. With your permission, they can also complete the application or talk on your behalf.

If you want someone to speak and act on your behalf on an ongoing basis, you can appoint them as your representative with My Aged Care. This will allow My Aged Care to talk to them about your aged care.

You can also get help from an advocate through the Older Persons Advocacy Network (OPAN). With your consent, an advocate can also speak to My Aged Care on your behalf.

Step Three

If your application is successful, an assessor will contact you to arrange a face-to-face assessment.

A face-to-face assessment can determine exactly what you need, and can even let you know about services you may not have thought about.

If you have immediate needs, My Aged Care may refer you directly to services before your assessment takes place.

How We Can Help

HOME CARE PACKAGES

Home Care Packages (HCP) are one of the ways that older Australians can access affordable care services to get some help at home. They are designed for those with more complex care needs that go beyond what the Commonwealth Home Support Programme can provide.

Home Care Packages can be an option if you need a coordinated approach to the delivery of your help at home - perhaps because you need help with many everyday tasks, or the care you need is more complex or intensive.


Commonwealth Home Support Programme

As you get older, everyday tasks can become more difficult. Even though you need help, you may still want to live at home. Asking for help doesn’t mean losing your independence or moving out of home.

The Commonwealth Home Support Programme (CHSP) helps senior Australians access entry-level support services to live independently and safely at home.

CHSP works with you to maintain your independence rather than doing things for you.
